Job overview

We are seeking a highly motivated and proactive individual to fulfil the role Patient Pathway Co-Ordinator with-in our busy Rheumatology Team at the University Hosptial Southampton, NHS Foundation Trust (UHS). Your job role will mean that you will build a good working relationship with the Consultants, the rest of the Admin Team and Nursing staff. 

ABOUT THE TEAM

You will be working within the Rheumatology PPC office with a small hard working team.

The post holder will be required to work full time hours (37.5 hours) and be available to work Monday – Sunday between the hours of 8am and 8pm (usual hours between 8am – 5pm Monday – Friday but you may be required to work an evening or weekend with prior notice).

Detailed job description and main responsibilities

WHAT YOU’LL DO

  • Implement all administrative processes along the patient pathway to ensure the patient’s journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.

  • Utilise computerised tracking systems to locate case notes and investigation results.

  • Liaise with Private Patient Co-ordinators where appropriate to ensure a seamless patient journey.

  • Ensure patient information is maintained and updated on the appropriate patient information system.

  • Monitor work lists for Choose and Book to monitor Consultant workflow and escalate to Team Leaders if necessary.

  • Ensure referral grading outcomes are logged on the appropriate patient information system.

  • Setting up clinics in liaison with consultants.

  • Managing clinic capacity in liaison with consultants.

  • Requesting and tracking medical records.

  • Capture and distribute clinic outcomes

  • Ensure follow up appointments and diagnostics are booked.

WHAT WE’RE LOOKING FOR

We are looking for a motivated and enthusiastic individual who is able to communicate effectively with patients via phone, email and face to face. You should have strong organisational skills and be used to juggling multiple tasks.

Qualifications

Essential criteria
  • Good standard of secondary education with literacy and numeracy skills demonstrable to GSCE pass level or equivalent standard.
  • RSA 2 word processing/typing or equivalent experience
  • Proficient in all aspects of Microsoft Office
Desirable criteria
  • Knowledge of medical terminology
  • Courses / further study attended to demonstrate evidence of personal development.
  • Diploma level qualification or equivalent experience.

Experience

Essential criteria
  • Experience as a medical or patient services secretary linked to a clinical service or customer focused environment
  • Administrative and organisational experience
Desirable criteria
  • Proficient in the use of all hospital computerised patient systems.

Aptitudes and skills

Essential criteria
  • Demonstrates clarity and breadth in use of verbal and written communications in relation to our patient group
  • Demonstrable ability to work under pressure
  • Proven ability to work to deadlines, prioritise and multi-task.
  • Able to learn and assimilate complex information quickly.
  • Willingness to undertake training
