An exciting opportunity has arisen for a continuing health care coordinator to join the integrated discharge bureau based at University Hospital Southampton. The successful applicants will be responsible for completing decision support tool documentation, gathering evidence against domains and chairing MDT meetings.
This team is currently undergoing a period of transformation, and there is plenty of opportunity to contribute to service redesign and service improvement projects.
As a continuing health care coordinator you will be based mainly on the wards, liaising with ward staff and patients but also within the Integrated discharge bureau where you would be working in partnership with Social Services and other members of the multi-disciplinary team.
You will be in regular contact with both local CCGs and may be required to attend panel to present applications. Training other staff groups regarding checklists, fast tracks and the full application process will be an important part of the successful applicants role. Also, you will able to contribute to an exciting pilot, working on a discharge to assess model for CHC and complex discharge patients.
As a continuing health care coordinator you will need to have a keen interest in the timely and efficient discharge process of this specialist patient group. You will be expected to be a confident communicator with excellent organisational skills.
You will be required to liaise with family members, patients and members of the multi disciplinary team on a regular basis, and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and often pressurised environment. The role will be full time, Monday to Friday, however it will be expected that you will work some weekends. This may vary due to the needs of the service.
You must have appropriate UK professional registration.
